The Private School Affidavit for the 2013-2014 school year is now posted on the CDE’s website. Please remember to file your affidavit online between October 1 and 15.

To find out if you need to file an affidavit and for step-by-step instructions that walk you through each line of the affidavit, please click here. The CDE made only minimal changes to the affidavit for 2013-14; we have noted these in our instructions.
