As you may know, HSLDA has created forms for our member families to use when notifying as either a "home study program" or private school. Our revised forms expressly protect your privacy. Box 94064 Baton Rouge, LA 70804-9064 RE: Application for Louisiana Home Study Program Dear Sir or Madam: As you may know, the Home School Legal Defense Association is a nationwide membership organization of more than 70,000 home schooling families. Our association is dedicated to preserving the fundamental rights of parents to direct the education of their own children. I am the attorney who serves our 613 member families in Louisiana. One of our member families recently forwarded to me a copy of your "Application for Louisiana Home Study Program or In-home Private School Program," revised July, 2000. I note that your form requests a social security number without identifying whether that information is optional. According to the Privacy Act of 1974, government agencies may not request a social security number without clearly specifying that that request is optional, or identifying the law which mandates provision of that number. I am advising our member families that they are not obliged to provide the social security number. Since you may not have been aware of the requirements of the Privacy Act of 1974, I will simply encourage you to modify your form in light of this new information.
